Mind Hamdard · Optional printable page

One concern. One next step.
The rest can wait.

A planning aid for adults, not a test or a treatment. Use only what feels useful. Leave identifying or sensitive details out, especially if someone else might see the page.

Tonight: keep it brief

What is the concern? A few words are enough.

Is there a realistic next action? If yes, what and when? If not, leave it unresolved tonight.

One quiet, manageable option for the next part of the evening:

Writing more is optional. Stop if the exercise increases distress. Do not use this page to set a strict time-in-bed or medicine schedule.

In the morning: notice, don't grade

Roughly when did I sleep and wake? What was the day like afterward?

Anything relevant: late caffeine, interruptions, work hours, medicine changes, or breathing symptoms?

One next step: repeat a helpful change, simplify the plan, or arrange an appointment.

Persistent or impairing sleep difficulty deserves assessment. Seek urgent medical help for serious new symptoms or if you cannot keep yourself safe. Do not drive or do hazardous work when sleepy.
