Mind Hamdard · Optional planning aid

Questions before the first conversation

An optional adult planning sheet. Use general labels instead of names or personal histories, particularly if a device, room, or printer is shared. It cannot guarantee privacy, judge a provider, or establish your legal rights.

Print and write on paper, or consider the prompts without making a record. There are no answer fields, submissions, or saved entries on this page. You do not need to complete it before asking for help.

Before booking: clarify the arrangement

Which contact or payment details might someone else see, and what do I need to ask the service about them?

What needs clarifying about qualifications, assessment, follow-up fees, cancellation, or the proposed format?

What do I need to understand about records, possible sharing of information, and urgent-contact procedures?

Write what the practitioner actually confirms. A promise from an online commenter is not confirmation of this service's arrangements.

For the first meeting: begin where you are

What is one moment that shows the difficulty, and what everyday activity has it affected?

What would make it easier to talk: a language preference, a question about family involvement, a slower pace, or a different setting?

What unanswered question should be discussed before another appointment is booked?

If privacy is interrupted, tell the practitioner. Ask how to pause or rearrange rather than assume an online session is private wherever it happens.

For immediate danger or an inability to stay safe, obtain urgent in-person help. An enquiry inbox may not be monitored promptly. This planning aid does not replace care or a safety discussion with a qualified professional.
